CAMPER HEALTH FORM

The following form is required by campers and is valid for 12 months after completion. Please provide a complete and accurate record of your child’s health history. If your child has any current or past health conditions that could affect his/her participation, please inform us below. Please refer to accompanying camp information or camp staff for specific activities planned for your camper’s program. Activities are designed to be challenging, but within the capability of any camper who is in good health.

PHOTO CONSENT & CELL PHONE POLICY

The following forms are required for all campers and must be completed before each camp program a camper participated in. It reviews key policies of camp regarding cell phone use, and camp's use of camper's images and likeness. Completion of these forms will help Camp Mardela ensure the safety and privacy of your camper during camp programs. The Cell Phone policy must be completed WHETHER OR NOT a camper owns a cell phone (by not owning a cell phone, that simply means they cannot bring it to camp, and therefore they are following the policy).
